About Benefits
At initial enrollment and at such other times as the Board of Trustees may determine, employees must elect between the following medical and prescription drug coverages:
Self-funded Medical and Prescription (PPO) and Kaiser (Deductible Plan), including its prescription drug benefits.
To receive the highest level of benefits, Active Self-funded PPO Plan participants may use an Anthem Blue Cross PPO network provider. Further explanation of the benefits may be found in the Kaiser brochures, which are available to you, upon request, from the Plan Administrator at no additional cost.
You or your provider may call the Trust Fund Office, United Administrative Services, at (408) 288-4400, to confirm your eligibility status or for information about the benefits payable under the Plan.
